Overview
Bekhterev’s disease, or ankylosing spondylitis, is a chronic rheumatic condition affecting the spinal joints, leading to inflammation of the ligamentous structures and the formation of ankylosis (fusion of the vertebrae). The sacroiliac joint and lumbar spine are most commonly affected, but as the disease progresses, other spinal regions may also be involved.
Diagnosis
Diagnostic methods include:
Spinal X-ray – Detects structural changes and ankylosis
Bone scintigraphy – Identifies areas of active inflammation
CT and MRI – Provides detailed imaging of spinal and joint involvement
Blood test for HLA-B27 antigen – Assesses genetic predisposition to the disease
Treatment
Management focuses on symptom relief, maintaining mobility, and preventing complications. Treatment options include:
Posture correction and physical therapy – Exercises to maintain flexibility and reduce stiffness
Medical treatment – Nonsteroidal anti-inflammatory drugs (NSAIDs), salicylates, disease-modifying antirheumatic drugs (DMARDs), methotrexate, and corticosteroids
Surgical treatment – Osteotomy (corrective surgery) or joint replacement in severe cases
